• On the left-hand side navigation menu under the services section click the Chain of Custody tab.

• Click the View Templates button.

• Find the specific template that you would like to use. Click the second gray button to Edit a Chain of Custody.

• The following General information fields can be edited:
  • Sampled By
  • QuickCheck
  • Case Investigation
  • From NY or CT
  • For Compliance
  • Primary biocide used in the potable water chlorine?
  • Primary biocide used in the non-potable water chlorine?
  • Project Identifier
  • PO Number
  • Tracking ID

• The following Sample information fields can be edited:
  • Order Number
  • Description
  • Sample Type
  • Water System
  • Date
  • Time
  • Tests

• Once all of the desired information has been edited, click the Save button.
